We are so pleased you are interested in enrolling at Big Lake Schools!
To enroll in any grade, K-12, please complete the K-12 Enrollment Packet. Other forms on this page may be optional or specific to a certain grade - please complete and submit any additional forms with the main enrollment packet.
For your convenience, the majority of our forms are fillable PDFs that allow you to type in the required information, then save or print. You can then mail, fax or email your complete packet to our district office.

K-12 Enrollment Pack (REQUIRED)
This pack contains all of the required paperwork for enrolling your student in Big Lake Schools. Make sure to read through the checklist carefully to make sure you don't need any additional forms to complete your enrollment.

Potassium Iodide (KI) Information (REQUIRED READ)
Please read regarding providing consent for KI to be administered if a radiological event occurs.

Application for Educational Benefits (OPTIONAL BUT RECOMMENDED)
All families are encouraged to fill out the Free and Reduced-priced School Meals application.

Transportation Form (REQUIRED IF USING BUSING)
This form should be used for a new transportation arrangement or to change an existing arrangement.

Student Digital Equity Survey
This survey collects information on student access to the Internet and electronic devices used for schoolwork in the student's home.

Fine Arts Elective Form (MIDDLE SCHOOL ONLY)
Select preferences for band, choir or other elective offerings.

Indian Education Form (ONLY IF INDIAN)
This form should be completed if student falls under Federal definition of Indian ethnicity/race.

Non-guardian Access Form (OPTIONAL)
Fill out and submit this form if you would like to grant access to your child's education data to a non-guardian (such as a step parent). He or she can then be given a parent portal. This form will also allow you to give permission for another person to pick up or drop off your child or allow to be listed as an emergency contact.

Data Privacy/Directory Information OPT OUT (OPTIONAL)
Parents who do NOT want directory information related to their child released to the public, used by local media, or used in district publications should submit this form.
